| Strathmore Day Opportunities provides a new approach a new approach to the way day services are provided for older people, including those with dementia.
There are three areas of support provided in Strathmore; Outreach and Carer Support, Community Group Support, or Building Based Support.
The service can be accessed by a referral form from: your GP, Nurse, Social Worker, Strathmore Dementia Team, Home Care Organiser, Reablement Service, Occupational Therapist, Perth & Kinross Council Early Intervention and Prevention Team. |
