What is it about?

We provide a specialist community service 5 days a week for individuals with a learning disability living in community settings and their own homes.

We work with individuals, their parents and carers, providing support and advice to enable individuals to live as independent a life as possible. We aim to reduce health inequalities, promote use and access of mainstream health services. Improve social inclusion by working collaboratively with the health and social care partnership, many other professionals and out side agencies. Thus giving our clients the opportunity to improve their physical and mental health.
