What is it about?

Hospital and Community Rehabilitation Support Services offering personalised support for people who have had a stroke.

Hospital Services:

1:1 or small group offering personalised support for people who have had a stroke, before moving on from Hospital, with e.g. communication and confidence

Community Services:

1:1 or small group offering support in the community after a stroke, to help you meet your agreed goals, e.g. accessing community groups, confidence building, communication support.

Affiliated Peer Support Groups:

For people who have had a stroke to meet weekly for social activities and outings. The groups are run by volunteers and affiliated to CHSS.
